New Patient Registration
Any person regardless of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition can apply to join the Practice providing they live within the Practice Boundary, are British citizens and the practice list is open.
If you are not sure whether you meet the criteria please ask at reception.
If you fulfil these criteria and wish to join please fill in the attached form (which can be downloaded here) and hand it in at the reception.
Once the paperwork has been checked we will accept you onto our list and apply to the Registration Department via an electronic messaging system to transfer your notes to the Practice.
If at any point your application is found not to fit our criteria we will be unable to accept the application and will inform you of the reason.
Accepted patients will be invited to see the practice nurse for a new patient medical.
Accessible Information Standard
We want to get better at communicating with our patients. We want to make sure you can read and understand the information we send you.
if you find it hard to read our letters or if you need someone to support you at appointments, please let us know.
We want to know if you need information in braille, large print or easy read. We want to know if you need a British Sign Language interpreter or advocate. We want to know if we can support you to lipread or use a hearing aid or communication tool.

New Practice Boundaries
- Motorway bridge at Bolton Rd towards Worsley Road North
- Little Hulton to Watergate Toll
- Ellenbrook, Boothstown and Moseley Common using Mort Lane and Sale Lane to mark the boundary up to the East Lancs A580 at "Aman's" Indian Restaurant
- Motorway bridge at Roe Green
- Motorway bridge at Wardley
- A580 traffic lights at "Aman's" Indian Restaurant
- Walkden Road up to St. Mark Church roundabout
- Please note that the boundary includes the whole of Boothstown.
Change of Details
