New Patient Registration

Any person regardless of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition can apply to join the practice providing they live within the practice boundary, are British citizens and the practice list is open.

You can check to see if you are within the practice boundary by using the postcode checker on the Practice Boundary page.

If you are not sure whether you meet the criteria please ask reception, you can do this online by completing the Ask Reception a Question form.

If you fulfill these criteria and wish to join please complete the New Patient Registration form.

Once the paperwork has been checked, we will accept you onto our list and apply to the Registration Department via an electronic messaging system to transfer your notes to the practice.

If at any point your application is found not to fit our criteria we will be unable to accept the application and will inform you of the reason.

Accepted patients will be invited to see the practice nurse for a new patient medical.

Accessible Information Standard

We want to get better at communicating with our patients. We want to make sure you can read and understand the information we send you.

If you find it hard to read our letters or if you need someone to support you at appointments, please let us know.

We want to know if you need information in braille, large print or easy read, if you need a British Sign Language interpreter or advocate and if we can support you to lipread or use a hearing aid or communication tool.

Please tell the receptionist when you arrive for your next appointment, or call us on 0161 790 8621 and speak to a receptionist. Alternatively, you can let us know online by completing the Contact the Practice form. We are here to help.

  • Motorway bridge at Roe Green

  • A580 traffic lights at “Aman’s” Indian Restaurant
  • Walkden Road up to St. Mark Church roundabout
  • Please note that the boundary includes the whole of Boothstown

Change of Details
